Rank One is the information management system for TAPPS (Texas Association of Private and Parochial Schools). Frassati Catholic High School is utilizing this dynamic platform to store student medical forms for the 2020/21 school year. Rank One is FERPA, PPRA and HIPAA compliant. Please follow the instructions listed below, and refer to the Parent Portal Tutorial.

Parents of new students are required by FCHS to set up an account on the Rank One Parent Portal, and upload a completed FCHS Medical History Form, and the FCHS Medical Exam Form. For all new or transferred students, a hearing screening must be performed, as mandated by the State of Texas. New or transferred students will need to have their physicals completed in their pediatrician's office (walk-in clinics do not have the equipment for hearing screenings). The medical professional will mark on the upper left hand side of the medical exam form if the student's hearing is normal, or if a referral is needed for an audiologist. Please make sure that this is not overlooked by the medical professional administering the exam, or the medical exam will not be accepted. Both medical forms need to be signed by a medical professional. The student's medical history form is filled out by the parent, but is still signed by the medical professional, who is acknowledging that he/she reviewed it before the exam. The medical history form also needs to be signed by the student and parent. Medical forms that are missing the required signatures will not be accepted. Please also complete the TAPPS Student Profile on the Rank One Parent Portal. The TAPPS Student Profile is where you will list emergency contacts, allergies, medical conditions and medications. Please follow the step by step instructions listed in the Parent Portal Tutorial. This must be completed before the school year begins!

You also need to complete and e-sign the TAPPS Student Profile, TAPPS Signature Page, FCHS Athletics Handbook Acknowledgment, and the Transportation Release. Parents and student athletes can also download the Rank One Public app. The Rank One Public app will be a primary means of communication from coaches and the Athletic Department, to parents & students. Coaches and the Athletic Department are able to send team messages, as well as alerts about practice or game changes. Your login/password for the Rank One Parent Portal and the app will be the same.
For new student athletes: the required forms must be completed before the school year begins, or before any on-campus summer camps, FAST Training, or fall sports practice. For returning fall athletes: the required forms must be completed before you can participate in fall sports practice, or in any summer on-campus camps or FAST Training. For returning winter or spring athletes: the required forms must be completed before you can participate in winter/spring sports practice, or in any on-campus summer camps or FAST Training.
